The State of Indiana and the Indiana Criminal Justice Institute are committed to protecting the privacy

necessary assistance from the Victims of Violent Crime Compensation Fund. This Privacy Policy

1. Claimant means a victim filing an application for assistance. The term also includes:
(1) a parent;
(2) a surviving spouse;
(3) a legal dependent; or
(4) a personal representative;
of the claimant.
2. User means the person using this website to submit an application.
3. Victim means an individual who suffers bodily injury or death as a result of a violent crime.
Information Collected
We may collect the victim’s name, address, telephone number, fax number and email address, together
with information about the crime, the perpetrator and any medical treatment received. Other
information that may be needed from time to time to process an application may also be collected as
needed.
Information Use
We use the information collected to process the application for which the victim/claimant is seeking
assistance. All reasonable precautions are taken to prevent unauthorised access to this information.
This safeguard may require the user to provide additional forms of identity should they wish to obtain
information about their account details.
Disclosing Information
We do not disclose any personal information obtained about the victim from this website to third
parties except when we need to do so in order to complete the review and investigation of an
application. We may also use the information to keep in contact with necessary parties to inform them
of developments associated with the claim. All health information is protected pursuant to the Health
Insurance Portability and Accountability Act of 1996 (HIPAA).
Changes to this Policy
Any changes to our Privacy Policy will be placed here and will supersede this version of our Policy.
We will take reasonable steps to draw your attention to any changes in our Policy at the time of the
change.
